Make a Claim

Select whether you have previously received a payment and/or concession card from Centrelink. If you do not know, please select the 'Unsure' option.

If you answer 'Yes', you will be asked to provide your Customer Reference Number (CRN). You can still answer 'Yes' if you do not know your CRN.

Customer Reference Number

This number is printed on your Concession Card and on all correspondence sent to you from Centrelink. Format of this number is 111 111 111A.

Proving your identity to Centrelink

When you claim a pension, benefit, allowance or service from Centrelink you must prove your identity if required. You must establish your identity by providing original documents (not photocopies) from Centrelink's approved list.

We may also use any documents you provide for proof of identity to help us verify your age, residency, income and assets if needed.

Name

Important Information

If you only have one legal name, e.g. 'Jane', you will be unable to continue your claim online as you must complete both Family and First name fields to be able to proceed.

Please call Centrelink on 132490 as soon as possible to discuss your claim.

You can also download the claim forms and information from this page, complete the forms and return them to a service centre. You can return your completed forms in person, by mail or by having a nominated person return them for you.

Legal Name

This is the name that appears on any of your official documents and legal papers. This will be the name shown and used by Centrelink

Title

Select your title from the dropdown box.

Family Name

You must enter your family or last name in this field. For example, if your name is Katie Jane Citizen, enter Citizen as your family name.

If you don't have a family name please see Important Information.

First Name

You must enter your first name in this field. For example, if your name is Katie Jane Citizen, enter Katie as your first name.

Middle Name

Suffix

A suffix follows a person's full name and provides additional information about the person. It indicates that the individual holds a position, educational degree, accreditation, office or honour. Examples of suffix could be:

Date of birth

This is the date you were born. Enter your date of birth as dd/mm/yyyy (that is, day, month, year).

Sex

Select either Female or Male.

Other names

Select the 'Yes' option if you have, or you have ever had, any other names.

Aboriginal or Torres Strait Islander

Please indicate if you are of Aboriginal or Torres Strait Islander origin.

This question will not affect your payment. If you do answer, the information will help us to continue to improve services to Aboriginal and Torres Strait Islander peoples.

If you are an Aboriginal or Torres Strait Islander and studying full time or part time, you can claim ABSTUDY instead of Youth Allowance or Austudy.

If you wish to continue claiming Youth Allowance or Austudy, select the 'Save' or 'Save and Continue' button.

To claim ABSTUDY you can telephone Centrelink on 13 2317 and ask to have an ABSTUDY claim form posted to you or you can pick up a form at any service centre.

Aboriginal

A person of Aboriginal descent who identifies as Aboriginal and is accepted as such by the community in which he or she lives.

Torres Strait Islander

A Torres Strait Islander is a person of Torres Strait Islander descent who identifies as a Torres Strait Islander and is accepted as such by the community in which he or she lives.

Australian South Sea Islander

An Australian South Sea Islander is a descendant of the original South Sea Islanders who arrived in Australia in the late nineteenth century and who identifies and is accepted as such by the community.

This is an optional question which you do not have to answer.
